The sucker looks like a thick pumped nipple with huge stainless steel rings piercing right through the nub. The inside of this one is large enough if you have thick worked nips or piercings or you wearing suckers for long play.
To get a good suction, smear a little water-based lube or spit around your nip before putting em on.
